Insurance: Turning Path Clinic is contracted with several insurance groups and will be happy to send a statement to your insurance as a courtesy to you. You must provide your insurance card at the time of service. You will be expected to sign an Assignment of Benefits.
It is up to you to know what your insurance benefits are, including the co-pay amount and the current years deductible amount. If there is a remaining balance after your insurance submits payment, you will receive a statement from Turning Path Clinic. Please understand that you are responsible for this balance.
Private Pay: If you do not have insurance, payment is expected at the time of service. We accept cash, checks and credit/debit cards. A 5% discount is applied for cash and check payments.
Any prices quoted to you are estimates only and may differ from the final billing.
Late Cancellation or "No-Show" Appointments: Turning Path Clinic reserves the right to charge up to $50.00 as a cancellation fee, if appointments are not cancelled at least 24 hours prior to scheduled appointment.
Additional Fees: Turning Path Clinic reserves the right to charge a $25.00 fee for checks returned for nonpayment due to insufficient funds.
Turning Path Clinic reserves the right to charge a fee for completion of forms.
Turning Path Clinic reserves the right to charge a fee for the printing and handling of medical records.
